Tirzepatide vs Semaglutide – What I Learned When Researching

Before starting my GLP-1 journey, I spent weeks researching the differences between semaglutide and tirzepatide. I thought I’d share what I learned in case it helps others who are trying to decide between the two.

Disclaimer: I’m not a medical professional – this is just information I gathered from research papers, forums, and conversations with my healthcare provider. Always consult your doctor for medical advice.

The Basics

Semaglutide (brand names: Ozempic, Wegovy, Rybelsus)

  • GLP-1 receptor agonist
  • FDA approved for diabetes (2017) and weight management (2021)
  • Available as weekly injection or daily oral tablet

Tirzepatide (brand names: Mounjaro, Zepbound)

  • Dual GIP/GLP-1 receptor agonist (works on TWO receptors)
  • FDA approved for diabetes (2022) and weight management (2023)
  • Available as weekly injection only

Key Differences I Found

1. Mechanism of Action

The biggest difference is that tirzepatide targets both GIP and GLP-1 receptors, while semaglutide only targets GLP-1. The dual action is thought to provide additional benefits for glucose control and potentially weight loss.

2. Weight Loss Results (from clinical trials)

  • Semaglutide: Average of 15-17% body weight loss in the STEP trials
  • Tirzepatide: Average of 20-22% body weight loss in the SURMOUNT trials

Keep in mind these are averages from clinical trials – individual results vary significantly.

3. Dosing

  • Semaglutide: Starts at 0.25mg, titrates up to 2.4mg (for weight management)
  • Tirzepatide: Starts at 2.5mg, titrates up to 15mg

4. Side Effects

Both medications share similar GI side effects (nausea, vomiting, diarrhea, constipation). From what I’ve read in forums and studies:

  • Some people tolerate one better than the other
  • Side effects often improve after the first few weeks
  • Slower titration can help minimize side effects

5. Cost & Availability

This is where things get complicated:

  • Brand-name versions of both are expensive without insurance
  • Insurance coverage varies widely
  • Compounded versions exist but quality varies
  • Shortages have affected both medications at various times

What Influenced My Decision

I ultimately went with semaglutide because:

  1. It’s been on the market longer (more long-term safety data)
  2. My insurance covered it better
  3. My doctor had more experience prescribing it

That said, I know plenty of people who’ve had great results with tirzepatide, especially those who didn’t respond as well to semaglutide.

Questions for Discussion

  1. For those who’ve tried both – did you notice significant differences?
  2. What factors influenced your choice between the two?
  3. Anyone switch from one to the other? What was your experience?

I’m genuinely curious about real-world experiences beyond the clinical trial data. Would love to hear your thoughts!
