Injection Site Reactions: What's Normal vs What's Concerning

Some reaction at injection sites is common. Let’s distinguish normal from concerning.

Normal Reactions

Immediately After

  • Small amount of bleeding
  • Slight stinging
  • Tiny bump at site
  • Mild redness

Hours to Days After

  • Small bruise
  • Mild itching
  • Slight firmness
  • Minor swelling

These typically resolve within a few days.

Potentially Concerning

Signs to Watch

  • Spreading redness beyond immediate area
  • Increasing pain over hours/days
  • Warm/hot to touch with redness
  • Pus or discharge
  • Fever
  • Large, hard lump that doesn’t resolve

These could indicate infection or severe reaction.

Minimizing Reactions

Technique

  • Proper site rotation
  • Clean injection technique
  • Room temperature medication
  • Appropriate needle angle

Site Care

  • Don’t rub the area after
  • Gentle pressure only
  • Keep area clean
  • Wear loose clothing over site

Rotation Importance

Why rotate sites:

  • Prevents lipodystrophy (tissue changes)
  • Reduces local irritation
  • Improves absorption consistency
  • Allows areas to heal

Rotation Strategy

  • Keep a log or use an app
  • Divide abdomen into quadrants
  • Include thighs in rotation
  • Don’t reuse exact spot for at least 1-2 weeks

Lipodystrophy

Long-term concern with repeated injections:

  • Hard lumps under skin
  • Indentations
  • Affects absorption
  • Prevented by proper rotation

When to Contact Provider

  • Signs of infection
  • Severe or spreading reaction
  • Allergic symptoms (hives, difficulty breathing)
  • Persistent large lumps
  • Any concern that worries you

Most reactions are minor - but always trust your instincts!
